چکیده

Case Report A 30-year-old woman was admitted with symptomatic bradycardia 2 weeks after undergoing a third ablation procedure for inappropriate sinus tachycardia (IST) in another centre. A permanent pacemaker was implanted. She re-presented 3 days later with swelling of both arms, neck, and head consistent with superior vena cava (SVC) syndrome. Venography showed bilateral brachiocephalic vein thrombosis extending to the SVC. There was no symptomatic improvement with therapeutic anticoagulation for 14 days; repeat venography demonstrated increased thrombus load. Systemic thrombolysis was administered (streptokinase 250 000 U bolus followed by 100 000 U/h over 24 h). Subsequent venography confirmed the resolution of the thrombus but persistent SVC obstruction. Acombinedprocedurewasperformedtostent theSVCandtorevisethepacing system.Theatrial leadwasremovedand theventricular lead was withdrawn into the subclavian vein providing access to the left subclavian vein and a channel through the occlusion (Figure 1A). A femoral approach was used to cross the SVC obstruction and place 16 × 40 and 16 × 20 mm self-expanding stents in the stenosed segment; the second stent was required as the initial 40 mm stent did not cover the occlusion fully (Figure 1B). A new atrial lead and the existing right ventricular (RV) lead were advanced through the SVC stent to the right atrium (RA) and RV septum, respectively. Symptoms resolved within 12 h. Thepatient did not showany recurrence of symptomsor signs ofSVCsyndrome and the pacemaker is functioning well at 12 months follow-up.

Malignant Lymphoma Demonestrating Sick Sinus Syndrome and Superior Vena Cava Syndrome

Reports which describe sick sinus syndrome due to malignant lymphoma have been rare and only eight cases have been reported until now. This is a case of sick sinus syndrome and superior vena cava syndrome secondary to invasion of occult malignant lymphoma of the lung in a 60 years old male. There were no symptoms or signs of malignancy before the first presentation with sick sinus syndrome. Pat...

Mediastinal Neuroendocrine Carcinoma of Unknown Origin Presenting with Superior Vena Cava Syndrome (SVCS): A Case Report

Primary neuroendocrine carcinoma (NEC) of the mediastinum is a rare type of carcinoma. According to the literature, only five cases of this condition have been reported so far. In this paper, we present a rare case of mediastinal NEC of unknown primary site. The patient was a 34-year-old man with mediastinal NEC, who presented with chronic dry cough and a right-sided mediastinal mass one year p...

Non-small Cell Lung Cancer with Superior Vena Cava Syndrome Effectively Treated with Implantation of Intravascular Stent and Chemotherapy A Case Report and Review of the Literature

Superior vena cava syndrome is caused by obstruction of the superior vena cava due to a variety of malignant and benign entities. The most common malignant entities are lung cancer and mediastinal tumor. Conventional treatments include bypass surgery, external beam radiation therapy, chemotherapy, and medical treatment. In this report, we performed an intravascular metallic stent deployment and...

Treatment of Tachycardia and Bradycardia in a Persistent Left Superior Vena Cava Patient Who Underwent Warden’s Procedure and Tricuspid Annuloplasty

Symptomatic supraventricular tachycardia is a common indication for radiofrequency catheter ablation treatment. Often, supraventricular tachycardia is associated with sick sinus syndrome, which requires the implantation of a permanent pacemaker. Underlying structural heart diseases such as complex congenital anomalies or a history of prior cardiac surgery can hinder successful procedural treatm...

Successful Treatment of Superior Vena Cava Syndrome Induced by Transvenous Permanent Pacemaker Implantation with Balloon Angioplasty

Superior vena cava (SVC) syndrome is a rare but potentially lethal complication of transvenous permanent pacemaker implantation. The pathogenesis is unclear, but thrombosis and fibrotic constriction of the vessels are believed to be responsible for this situation. Multiple, severed, and previously infected leads may increase the risk of SVC syndrome, but are not prerequisite for it. Anticoagula...
